Creating a smart form - Logic Fields

This video demonstrates how to set up five types of smart form fields in Q-Hub: conditional fields that branch logic, task creation with assignments, calculation fields that compute values, action groups that offer form options, and read-and-sign fields for document verification. You'll see how each field type works both in configuration and during live data entry.

Let's take a look at our logic input types. First of all, we've got conditional. Now, this is a field that can lead to more actions. So, for example, we can put no, we can change the color to red, we can add another action, give the user option of yes. So if they select no, we can put no further action is required by toggling the not applicable box. And if we click yes, we can then ask for more information.

We can also create a task. So first of all, we can allow the user to select the task owner by a preset list of users, or we can allow the user to choose the task owner from anyone on the platform. We can then give the task a title, select the priority, give the task a description, not forgetting to tick the green tick when we're done. Then we can add an attachment if necessary, and at the bottom we can add more task fields or subtasks. And on the bottom bar here, you do have the option to use task templates if these have been input already.

Let's now take a look at our calculation field. So to use this type of field, we need at least two, either weighted list or number fields. As you can see, we have one of each here. So we can go ahead and add the title, then just set up our calculation. We're going to click the pen icon, and in the top box, we can either type a dollar sign and select which fields we want to use, or as you can see, the system pre-generated it for us. So we'll simply press close.

Let's take a look at our Action Group field type. So here, we can pre-build actions to offer to the user to take. So we'll use start a form as an example. We give the action a title and then select a form, click save. And then, along the top, you can see it gives us the option to make the field mandatory. And then we'll click done when we're ready.

Let's now look at the read and sign field. So from here, we can ask the user to read and sign a specific document, or we can pick a folder. Let's take a look at document first. So simply select the document you wish to use, or we can pick from a portal folder, or alternatively upload a new file. If we go back, the process is the same with the folder select. We can either choose from our document library, and this will show up as individual documents so that the user can go through, read and sign to say they have read each one. We can then select whether we require a signature at the end.

Let's see those fields in action on our live entry. So if we click add an entry, then at the top we have our conditional field. So if we select no, that didn't require anything further. If we select yes, we can see the actions that we've pre-populated. Then, for our calculation field, let's go ahead and select a value. We'll input a number, and this calculates our training score. There with our action field, we can see we need to fill in a separate form. And once we open this secondary form, along the top it creates a link for us to go back to the original form that we were working from. Under our read and sign field, as you can see, there are multiple documents. The option to tick to see we've read a document won't actually highlight until we've opened the document. And now we can see this box is now highlighted that we've read all documents required, and then we can input our name and create a signature.
